MEMBER COMMUNITY FUND

Applications for our Perfect Pet Club Member Community Fund are invited when a new Member joins us. If your assessment for award eligibility has not been approved prior to any veterinary care being carried out at a veterinary practice other than Animal Magic Family Health & Wellness Centre, the maximum grant award payable for veterinary services used is 60% and a total exemption will be applied to any pre-existing condition.

Eligibility Criteria for Grants Up to 40% of the Total Cost

​​​

• You must be a Perfect Pet Club Member outside of your 28 day exemption period

• The condition has been accepted as an inclusive pre-existing condition

​

Eligibility Criteria for Grants Up to 60% of the Total Cost

​​​

• You must be a Perfect Pet Club Member outside of your 28 day exemption period

• The condition must not be exempted from your Membership

• Individual Income less than £30,000 or Household Income (at least 2 adults) less than £60,000

• Savings of less than £3,000

• The pet is not used as a working or breeding animal

• Proof must be provided to show that you meet all eligibility criteria

Eligibility Criteria for Grants Up to 80% of the Total Cost

• You must be a Perfect Pet Club Member outside of your 28 day exemption period

• The condition must not be exempted from your Membership

• Individual Income less than £15,000 or Household Income (at least 2 adults) less than £30,000

• Savings of less than £3,000

• The pet is not used as a working or breeding animal

• Proof must be provided to show that you meet all eligibility criteria

Eligibility Criteria for Grants Up to 100% of the Total Cost

​​​

• You must be a Perfect Pet Club Member outside of your 28 day exemption period

• The condition must not be exempted from your Membership

• The Treatment must be carried out at Animal Magic Family Health & Wellness Centre OR you are applying for an additional grant to cover the difference between a previous award because you are struggling financially. We also issue 100% grant awards to Members who usually visit Animal Magic Family Health & Wellness Centre and need to be referred elsewhere because the care cannot be provided directly by us for any reason.

• Please note: we only award 100% grants to Perfect Pet Club Members who fall in the 40%, 60% or 80% categories to cover costs incurred at alternative veterinary practices in exceptional circumstances

Application & Proof of Income

 

​​​You should apply for a pre-approved grant award assessment as soon as you have joined Perfect Pet Club:

​

• Send a personal statement by email to explain your current financial situation, and if you expect this to change within the next 12 months

• Last 3 months bank statements for every household member

• Full history from the veterinary practice your pet is registered with

• Any pet insurance policy documentation

Approval and Award

​

• You will receive confirmation of your pre-approved grant allowance within 7 days of receipt of your application

• Any approved grant award will be paid within 28 days of receipt of invoice following any veterinary treatment administered; either directly to the veterinary practice involved or if already paid, to the specified Member bank account.

• Please remember that all prescription medication (to include flea and worming treatment and any medication prescribed to non-exempted Premier and Elite Members) must be dispensed by Animal Magic Family Health & Wellness Centre, unless we have pre-approved a written prescription fulfilment elsewhere

• If your financial situation changes at any point during the term of your Membership, it is important that you let us know immediately

Services Eligible for 100% Grant Award​​​

​

• Annual vaccinations

• Monthly flea and wormer prescriptions (these are dispensed directly from Animal Magic Family Health & Wellness Centre at the end of each month, unless the Member has received approval for a written prescription to be fulfilled elsewhere)

• Neutering (where possible, this procedure should be carried out at Animal Magic Family Health & Wellness Centre in order to avoid unnecessary expense for our organisation)

• Appointments and procedures carried out at Animal Magic Family Health & Wellness Centre (subject to Membership status, availability and practicality of the visit)

THE ANIMAL MAGIC FAMILY

EMERGENCY CARE FUND

​

Applications for our Emergency Care Fund are open to Perfect Pet Club Crisis Care Members who reside in the UK. Percentage contribution grants for urgent procedures resulting from traumatic causes of injury or sudden onset apply to conditions such as:

​

• Pyometra

• Foreign body removal

• Interception

• Splenectomy

• Enucleation

• Limb amputation

• GDV​

• IVDD

Eligibility Criteria for Grants Up to 60% of the Total Cost

​​​

• You must be a Crisis Care Member outside of your 28 day exemption period

• Individual Income less than £30,000 or Household Income (at least 2 adults) less than £60,000

• Savings of less than £3,000

• The pet is not used as a working or breeding animal

• Proof must be provided to show that you meet all eligibility criteria

Eligibility Criteria for Grants Up to 80% of the Total Cost

• The condition must not be exempted from your Membership.

• The Treatment must be carried out at Animal Magic Family Health & Wellness Centre OR you are applying for an additional grant to cover the difference between a previous award because you are struggling financially. We also issue 100% grant awards to Members who usually visit Animal Magic Family Health & Wellness Centre and need to be referred elsewhere because the care cannot be provided directly by us for any reason.

• Please note, we only award 100% grants to cover costs incurred at alternative veterinary practices in exceptional circumstances.

Application & Proof of Income

• Send a personal statement by email to explain why you are unable to afford the cost of treatment

• Quotes from three veterinary practices (in a time sensitive emergency a quote from one practice is acceptable)

• Last 3 months bank statements for every household member

• Full history from the veterinary practice your pet is registered with

• Any pet insurance policy documentation

• It is advisable to provide us with this documentation at the time of joining, and again when any updates apply. This will eliminate the grant approval process in the event of an emergency.

Approval and Award

​

• If you have not already been pre-approved at the time of joining or re-assessment, we aim to provide you with approval within 1 hour of your call

• Your grant award will be paid within 28 days of approval

Applications to The Animal Magic Family Community Fund are open to all and awards are discretionary based on your application. If you haven't already, please ask your veterinary practice to consider allowing you to repay the costs of treatment with an affordable payment plan. Many practices are willing to do this to help you access the care your pet needs.

• You will receive the outcome of your application by email within 48 hours of receipt

• If approved, your grant award will be paid within 28 days of receipt of invoice following any veterinary treatment​​
